• Any government issued identification card
• Taxpayer Identification Number

To claim through your representative, please bring the following:

1. Authorization letter
2. ID and TIN of winner (government issued)
3. ID of representative (government issued)

Winners are requested to claim their prize on or before February 25, 2010.
